The National Heart Foundation of Australia ABN 98 008 419 761 (we, our, us, the Heart Foundation) is collecting your personal information through the Moodle online learning platform registration form including your name, email, profession, postcode and professional body registration number.

We have collected this personal information for the purposes of allowing you to register on the Moodle platform and access the resources, conduct surveys and reporting on your use of the platform, send you our Heart Health network newsletter and to keep you informed about Heart Foundation Healthcare Program related activities.

Your information may be disclosed to our third party software provides (including Moodle Pty Ltd (ABN 55 116 513 636 and My Learning Space ABN 45 536 763 230, and other implementation/support partners) for the purposes of facilitating registration and access to the website content.

Providing us with your personal information is not required by law, but if you do not provide us with this personal information, we may be unable to register you on the site and provide you with the learning resources.
